Delgado walked four straight times before doubling to deep right field to score David Wright and put the Mets ahead 3-2.
The win was the third straight for the NL East leaders, who added run-scoring hits by Shawn Green, Ruben Gotay and Jose Reyes in the inning to knock the Cubs out of first place in the NL Central Division.
All four runs came off Cubs closer Ryan Dempster, who in May gave up five runs in one-third of an inning as the Mets rallied from a 5-1 deficit in the ninth inning.
"We don't think about that," Delgado told reporters. "We think about getting one in, and then another one, and then another one."
The Mets had 11 walks in the game, seven of them by Cubs starter Carlos Zambrano, who was seeking his 15th win of the season.
Zambrano gave up only two hits, including a sixth-inning homer by the Mets' Ramon Castro, before leaving with heat cramps.
Chicago's Derrek Lee evened the score at 1 with a home run in the bottom of the sixth.
Both teams scored a run in the seventh to tie it at 2-2. Marlon Anderson singled in the Mets' run and Jason Kendall doubled for the Cubs' score.
*San Francisco's Barry Bonds went 0-for-4 with a walk to remain one short of Hank Aaron's all-time home-run record of 755 as the San Diego Padres edged the Giants 4-3 in 10 innings on Scott Hairston's homer.
